Pathfinders help people with developmental disabilities to create a Life Plan. Life Plans are tools that aid people in planning what services and supports they need to live successful lives.
More importantly, Life Plans help people learn more about themselves. Plans also help support persons and Service Coordinators learn more about how the person wants to be supported as well as what is working and not working in a person?s life. Life Plans are a great bridge between the individual and the people who support them.
What is a Life Plan?
A Life Plan is a complete review of the life of the person with a developmental disability, with an emphasis on how a person can be supported in achieving their goals and dreams. Life Plans are designed to be complimentary to the IPP. They give service providers a clear picture of the wants and needs of the person, which in turn creates a more effective system of support. Life Plans are updated annually.
